Travel Vaccinations

If you require any vaccinations relating to foreign travel you will need to complete a travel risk assessment questionnaire. Once you have completed the questionnaire please return it to the practice. The nurse will then review the questionnaire and decide which vaccinations you will need. The practice will then contact you to book an appointment.

Please aim to return the questionnaire at least 6 weeks before you are due to travel.
