New Patients

Orchard Family Practice is open to register patients.    Please check our boundary map below to make sure that you fall within our practice catchment area before you try and register with us – we will ask to see proof of address before we can accept you on to our list.

If you fall within our catchment area, please ring the Surgery on 01634 568200 and the Reception team will add you to the new patient list.  You will be asked to ring us back in a couple of days to confirm that you are able to join us.

Once you are accepted as a new patient, you will need to collect a new patient pack from Reception and complete all the forms inside and return them to the Practice.

Due to increasing capacity issues – these forms need to be collected and returned to us within 2 weeks otherwise your registration will be cancelled.

Once we have received your patient pack, your registration will be processed.  You will not be able to make an appointment until we have imputed your information onto the system.
